Gist of the Scheme | Issue of National identity cards to Differently Abled. National Identity cards are issued by District Differently Abled Welfare Officers based on the Disability Certificate. |

Eligibility Criteria | Should have a minimum of 40% disability |

Whether form of application is prescribed. | Yes. Available with District Differently Abled Welfare Office. |

Certificates to be furnished | Medical Certificate should be obtained from the competent Medical authority (ie.) concerned Specialist. In case of multiple disabled person, certificate should be obtained from Medical Board. |
