திட்டத்தின் சுருக்கம்

கை, கால் பாதிக்கப்பட்ட  மாற்றுத்  திறனாளிகள்  மற்றும் இரண்டு கைகள் மற்றும்  பாதிக்கப்பட்டவர்கள் சுலபமாக சென்று    வர இலவசமாக சக்கர நாற்காலி வழங்கப்படுகிறது.

திட்டத்தில் பயனடைய தகுதிகள்/நிபந்தனைகள்

12 வயதுக்கு மேற்பட்டு,கை, கால்கள் பாதிக்கப்பட்டவர்களாக  இருத்தல்  வேண்டும்.அந்த மாவட்டத்தில் வசிப்பவராக இருக்க வேண்டும்.

தனிப்பட்ட விண்ணப்பப் படிவம் உள்ளதா?

ஆம்.சம்பந்தப்பட்ட மாவட்ட மாற்றுத்திறனாளிகள்
நலஅலுவலர்களிடம் உள்ளது.

இணைக்கப்பட வேண்டிய சான்றிதழ்கள்

தேசிய மாற்றுத் திறனாளிகள் அடையாள அட்டை,

விண்ணப்பங்கள் பெற்றுக் கொள்ளும் அலுவலர்

மாவட்ட மாற்றுத் திறனாளிகள் நலஅலுவலர்

உதவிகள் வழங்கப்படும்போது காலதாமதம் ஏற்பட்டால் அணுக வேண்டிய அலுவலர்

WHEEL CHAIRS

1.	Gist of the Scheme	Wheel Chairs are given to the needy identified and deserving Orthopaedically differently abled and Paraplegic persons for their easy mobility.
2.	Eligibility Criteria	The candidate should be  affected with both lower limbs and resident of the District/area. Having disability of 75% and above.
3.	Whether form of application is prescribed	Yes. Application are available with concerned District Differently Abled Welfare Officer.
4.	Certificates to be furnished	National Identity Card for Differently Abled
5.	Officer to whom the application is to be submitted	District Differently Abled Welfare Officer
6.	Grievances if any to be addressed to	Principal Secretary / State Commissioner for the Differently Abled, State Resource cum Training Centre Campus, Jawaharlal Nehru Inner Ring Road, K.K. Nagar, Chennai – 600 078. Ph: 044-24719947 / 48 / 49
